# TMS prior-authorization readiness checklist

Check paperwork readiness, not treatment eligibility. Mark a task only when the clinic has confirmed it. Do not enter diagnoses, medicines or health records. The checklist follows topics in WPS Medicare LCD L34641.

Use the tool on the [web page](https://tmsclinics.fyi/tools/tms-authorization-readiness). It works without JavaScript.

## How to use it

1. Enter i have the current payer policy and the clinic has checked it.
2. Enter the clinic has the assessment and treatment order.
3. Enter the clinic has the previous-care records required by the policy.
4. Enter the clinic has any required psychotherapy and symptom-measure records.
5. Enter the clinic has documented the proposed device, protocol and safety review.
6. Enter network status, request route and planned visits are confirmed.
7. Read the result.

## How it works

CMS/WPS LCD L34641, revision effective 26 March 2026, https://www.cms.gov/medicare-coverage-database/view/lcd.aspx?lcdid=34641, fetched 2026-10-29. Coverage topics include confirmed severe major depressive disorder, medication history or intolerance, psychotherapy, symptom measures and a psychiatrist’s order and supervision. The clinical team must interpret the exact policy wording.

This tool asks whether paperwork exists. It does not ask whether you meet diagnostic or medical-necessity rules. Payer policy, network status and request procedures must be checked separately. Marking every box produces a prompt for clinic review, not an approval score.

Medicare LCDs are local. Private insurers and other countries use different rules. Give clinical records to your treating team through a secure channel.

> Not a medical assessment or a coverage decision. Confirm suitability, protocol and cost with a qualified clinician.

## Questions

### Does a complete checklist mean I qualify?

No. Your clinician assesses suitability and the payer decides coverage. The checklist only tracks document tasks.

### What if I use a different insurer?

Ask for its current written policy. Use this list only to start the paperwork discussion.
